Become a new patient at BMC
As of May 14, 2024, we have a wait list for new family medicine patients. Feel free to complete paperwork below. We will contact you as soon as we have appointments available.
Step 1: Fill Out Health History Forms
- Fill out a New Patient Packet for family practice patients.
- If you are looking for mental health counseling, please fill out the Counseling Intake as well.
- Forms also available for pick-up inside the front door at Blue Mountain Clinic.
- If you are in need of abortion care, please call us at (406) 721-1646
Step 2: Submit your completed forms
- If you are unable to complete your packet online, you can:
- Mail them to 610 N. California St, Missoula, MT 59802
- Drop them off at BMC’s location. There is a secure dropbox located in the front entryway.
- Fax them to 406.543.9890
- Scan and email them to frontdesk@bluemountainclinic.org
Step 3: BMC will call you to schedule
- We will contact you via phone to schedule your first appointment. If we are unable to match you with a provider, you will be notified via mail.
